Photobiomodulation explained
Class IV K-Laser therapy, also called high-power laser therapy or photobiomodulation (PBM), uses focused light energy at therapeutic wavelengths to stimulate healing deep within tissue. Class IV refers to the laser power class: these devices deliver 7 to 15 watts of output, far higher than the 0.005 to 0.5 watts of older Class III "cold" lasers. The higher power lets the light penetrate 4 to 5 centimeters into muscle, tendon, joint capsules and nerve tissue without heating or damaging the skin.
The biology behind it: light at red and near-infrared wavelengths (typically 800 to 1064 nanometers) is absorbed by a mitochondrial enzyme called cytochrome c oxidase. Class IV K-Laser therapy has an excellent safety record when performed by a trained licensed provider following manufacturer protocols. The vast majority of patients experience no side effects at all, which is one reason laser therapy has become a preferred option for patients who cannot tolerate medications or injections. The most common sensation during a session is mild, gentle warmth as the laser energy is absorbed into the tissue. After a session, a small number of patients report a brief, mild increase in soreness for 12 to 24 hours, which usually means the inflammatory response is reorganizing as healing accelerates.
Serious risks are very rare. The two most cited concerns with Class IV laser therapy are skin burns and eye injury, both of which are prevented by trained technique. Eye safety is non-negotiable: every person in the treatment room, patient and provider, wears wavelength-specific protective glasses for the entire session. Class IV lasers must never be looked into directly.
